In short: professional whitening lifts stains from tea, coffee, tobacco and paan and can lighten teeth by several shades safely. It is not permanent, it will not change the colour of crowns or fillings, and it is not a substitute for a cleaning. Done properly it does not damage enamel.

Two different problems that look the same

Extrinsic stain — on the surface

Pigments from tea, coffee, red wine, tobacco, paan, supari and strongly coloured food build up in the film on the outside of the enamel. This responds beautifully to a professional scaling and polish — often the teeth look dramatically better after a cleaning alone, with no whitening needed at all.

Intrinsic discolouration — within the tooth

The dentine underneath the enamel is naturally yellow, and it darkens with age while enamel thins. Some discolouration is built in: tetracycline banding from antibiotics taken in childhood, fluorosis from excess fluoride during tooth formation, or a single tooth that has darkened after its nerve died from an old injury. This is what bleaching addresses.

Which one you have determines what will actually work — and it takes an examination to tell. Bleaching heavy surface stain without cleaning first gives a poor and patchy result.

How whitening works

The active ingredient is hydrogen peroxide or carbamide peroxide. It penetrates the enamel and breaks down the large pigment molecules trapped in the tooth into smaller, colourless ones. It is not paint and it does not strip a layer off — the enamel structure is unchanged.

The options

In-clinic whitening

A high-concentration gel is applied after your gums are carefully isolated with a protective barrier, in two or three cycles across a single appointment of roughly 60 to 90 minutes. Results are immediate. This is the right choice if you have an event coming up or simply want it done and finished.

Take-home trays

Custom trays are made from impressions of your teeth, and you wear them with a lower-concentration gel for a set period daily over one to two weeks. It takes longer but often gives a more even, longer-lasting result, and you keep the trays for occasional top-ups later.

Combination

An in-clinic session for the immediate change, plus trays to extend and maintain it. This is what we most often recommend for heavy staining.

Internal bleaching

For a single dark tooth that has had a root canal, the bleaching agent is sealed inside the tooth itself for a few days. It is the only approach that works well for that specific problem.

What whitening cannot do

  • It does not change crowns, veneers, bridges or fillings. These stay exactly the shade they were made. If you have a white filling on a front tooth and you whiten, that filling will suddenly look darker than the tooth and may need replacing afterwards. Plan the sequence: whiten first, then match the new restorations.
  • It does not work well on grey teeth. Yellow and brown discolouration responds best; grey — particularly tetracycline grey — responds slowly and incompletely. Veneers may be the honest answer there.
  • It is not permanent. Expect one to three years depending on your diet and whether you use tobacco.
  • It does not whiten indefinitely. Every tooth has a natural ceiling. Chasing an unnatural brightness gets you sensitivity, not whiter teeth.

Is it safe?

Professionally supervised whitening has a long safety record. Temporary sensitivity to cold during and shortly after treatment is the main side effect, affecting a good proportion of patients and settling within a few days; a desensitising toothpaste for a couple of weeks beforehand reduces it considerably.

The risks come from doing it unsupervised. Over-the-counter kits with ill-fitting trays leak gel onto the gums and burn them. Charcoal powders and DIY baking-soda-and-lemon remedies are abrasive or acidic and physically wear enamel away — which is irreversible, and exposes more of the yellow dentine underneath, making teeth look worse over time. We would rather you came in and were told you do not need whitening than have you sand your enamel off at home.

We do not whiten during pregnancy or breastfeeding, or for children whose teeth are still developing. Untreated decay and gum disease must be dealt with first — peroxide reaching an open cavity is extremely painful.

Keeping the result

  • Avoid strongly coloured food and drink for the first 48 hours, while the enamel is more permeable — this window matters more than anything you do later.
  • Use a straw for cold coloured drinks.
  • Rinse with water after tea or coffee.
  • Tobacco and paan will undo the result faster than anything else.
  • Keep six-monthly cleanings; top up with your trays when you notice it drifting.
